[interprocess] non-ASCII characters in headers

Hi Ion, There are a few non-ASCII characters is the interprocess headers, which cause Boostbook to reject a few of the XML files and miss a few pages from the reference documentation (such as the flat_map documentation). Daniel -- My opinion is that all of those changes are fine - except for the one where you changed "Dietmar Kühl"'s name. There's a bug (and a feature request) open against non-ASCII characters in boost source files (and headers), and I've been fixing some of them; but I have refrained from changing people's names.
