It looks like there were patches which got lost when we moved
to trac/svn.  Anyway,

1) This is not a widely needed feature.
2) Breaking backwards compatibility is out of the question, IMO.
3) Preserving backwards compatibility increases the compile time cost of 
using mpl::pair.
