[boost] [acc][test] missing include in boost/test/progress_monitor.hpp

Recent tests from FFT-aCC for HP aCC 6.00 seem to point at a missing #include <ostream> in boost/test/progress_monitor.hpp: http://tinyurl.com/8v445 This is currently blocking many many testcases in FFT-aCC.
