boost/graph/kolmogorov_max_flow.hpp libs/graph/doc/kolmogorov_max_flow.html

These two were deprecated a few versions ago and were removed in the trunk, but I considered it a "major change" that I missed the deadline for.

boost/pending/integer_range.hpp

This is probably safe to remove since I don't see any uses in Boost outside documentation (and I don't believe it was ever used outside Boost). -- Jeremiah Willcock
