[boost] Thread->join() deadlock

FYI: The limitation page (http://www.boost.org/doc/html/threads/implementation_notes.html#threads.impl...) does not mention that one may not invoke thread.join() from within DLLMain because this too will lead to a deadlock (due to ExitThread()'s behavior).
