It's moderated-on-first-post /and/ you have to be subscribed or your post will never get to the moderation queue.
Checked that last night already, either user malfunction or list queue gremlins, I guess.

One possible gremlin is that a moderator meant to click "accept" but accidentally clicked another button and didn't realize it. The poster should try again. --Beman
