[boost] [python] patch modules_init.hpp to expand macros

This patch replaces use of ## and # with BOOST_PP_CAT and BOOST_PP_STRINGIZE. This is needed so that python module names can be programmatically generated. https://svn.boost.org/trac/boost/ticket/3843
