Something's not right here. It should be generated. Here's an example from the python tutorial:

Is it conditional on something inside the .qbk file? By the way, your own quickbook documentation doesn't seem have a top-level index either ;-)

Ah yes. I understand now. The generation of the top-level index depends on what you are generating. It's a BoostBook thing that I don't quite understand yet. It somehow depends on the document-type. An article will not have a top-level index; a library will. If you really want to know why, you should probably ask the boost-docs list. I don't know the answer. Anyway, I cross posted to the list.
